This is a quite complicated survey about dreams, done online.  You can review results to date and engage in discussion, by posting and replying to messages (not much there at the moment but maybe ELI can change that).

This is a good but advanced summary report of a survey.  The survey is included at the end of the document.  You may be able to use it in an MMC lesson to discuss survey development, how all the questions work together to form a ‘picture’ and the relationship of the text to the survey. http://www.facilities.usyd.edu.au/projects/docs/transport_survey.pdf

 This is an example of a simple survey about public transport.  It shows appropriate questions to establish an integrated ‘picture’ of the issue - public transport.

 This is an example of a detailed survey with results for the National Sports Federation (US).  The link takes you to the summary results page with questions, statistics and graphs.  Click on ’view details’ and you can look through the individual responses.  
You may be able to use this as the basis of a research and analysis activity.  Deconstruct the survey and how the results are summarised and presented.
